Our goal is to provide a premium service to you to maximize your outcome to get you back to your normal activities sooner. We focus on quality of care with one-on-one treatment.


Each patient will receive their own personal plan of care catered toward the goals at the evaluation visit.


Each appointment is a one-on-one session.

Our physical therapist believe it is important to spend quality time with our patients the whole time they are here in order to see greater results and ensure the patients understand how to do their exercises properly to prevent further injury.


How to get started on your Evolution journey:

  • Call or schedule online for an appointment for an evaluation.

The first visit with us will be an evaluation. At the evaluation, we will address the reason that brought you in for physical therapy, go over any imaging results or other physical therapy you have done in the past and discuss if physical therapy makes most sense for your care, if it does, we will create a plan and set you up with some follow ups according to the plan of care.

  • We will send intake papers to you to fill out through a link via email and text message.
  • If you are unable to complete the intake papers prior to the appointment, please arrive about 15 minutes prior to your appointment time to allow time to complete in the office.
  • You can expect each appointment to be about 45 minutes.
  • Bring your health insurance card and photo id.
  • Wear comfortable clothes that allow for easy movement.
  • Bring any imaging or notes from other doctors regarding the issue you are coming in for if you have them.
  • If you have any braces, crutches, and splints, bring those with you.
  • If you are a Medicare patient, please have your doctor write you a script. These may be faxed to 410-628-0542 or you can bring this with you at your first appointment.
  • If you have an HMO or Medicaid plan, please have your doctor write you a referral. These may be faxed to 410-628-0542 or you can bring this with you at your first appointment.
  • We do accept insurance but not all. Please check with us if you are unsure if your plan participates with our office.
